Navigating the Shift to Large Scale Combat Operations
The transition from counterinsurgency operations to Large Scale Combat Operations (LSCO) represents a fundamental shift in how the Army prepares for conflict. In past environments, the "Golden Hour" for evacuation was often a standard expectation. However, in an LSCO environment, resources will be constrained, and communication may be limited by electromagnetic interference. We must now prepare for prolonged field care scenarios where medics and providers may have to hold patients for 24, 72, or even more hours before evacuation becomes possible.
The Foundational Importance of Clinical Skills
High-tech solutions are secondary to the foundational clinical skills of the individual provider. Whether in a hospital or an austere environment, a physician’s ability to synthesize information and negotiate a treatment plan with a patient is the same skill set required to lead a strategic organization. For the front-line medic, survival depends on the ability to think critically in the face of adversity. Mastery of these basic crafts is what allows a provider to solve the "unsolvable problem" when standard protocols fall short.

Today, the Army is aggressively working to integrate paramedics into medical treatment facilities and operational units across the board. Biography
Brigadier General Lance C. Raney, commissioned into the U.S. Army Medical Corps in 1993, has served as the Deputy Surgeon General of the U.S. Army since June 2025. A Texas native, he earned his MD from the University of Arkansas and completed a Family Medicine residency at Eisenhower Army Medical Center. His distinguished operational background includes deploying to Iraq as Brigade Surgeon for the 172nd Infantry Brigade.
Demonstrating a strong trajectory in medical leadership, BG Raney commanded both Raymond W. Bliss Army Health Center and Womack Army Medical Center. His strategic impact expanded through roles like Chief of Consultants to the Army Surgeon General and clinical operations chief for the DoD COVID Testing Task Force. As a General Officer, he served as FORSCOM Command Surgeon. His robust career offers excellent context for archiving military medicine history. He holds many prestigious commendations including the Bronze Star Medal and Combat Medical Badge.
